I have a 2000 Avalon sometimes fails to start. When the key is turned
there is one click. On the second try it usually starts. The local
Toyota dealer says he wants $100 to diagnose the problem, but being
intermittent he still wants the money even if he can't find any
problem. How likely is it to be a failing starter solenoid? What
should that cost?

The symptoms you are describing are pretty typical for failing starter
solenoid contacts. If you are willing to do some work yourself, you can fix
it for under $40. If you want to replace the starter yourself, the price
will be higher.

More likely you need to clean all of the battery connections.
